Required Forms Include:

- A copy of child's birth certificate
- Completed Registration Form from the AZ Depart ment of Health Services
- A copy of your child's current immunizations (or the Religious Belief Exemption Form)
- Signed family handbook

Additional requirements for enrollment:

- Completed in-person tour of UPSIDE
- Family interview
- $65 registration fee
